About The Position

The Community Health Worker provides care coordination and insurance navigation as part of a collaborative team with Patient Access, Revenue Cycle, Quality, Population Health, Health Information Management, and clinical staff. They work directly with patients, many of whom are high risk, along with their families and NoCo's primary care providers, to keep care smooth and continuous. The role includes making patient outreach calls, providing transportation and other enabling services, and helping close gaps in care. The CHW also helps patients get and keep health insurance; this includes helping with Medicare, Medicaid, Essential Plan and Marketplace applications and renewals, explaining new work requirements, and making sure paperwork is submitted on time. They serve as the point of contact, advocate and information resource for patients, care teams, families and caregivers, payers, and community resources. The CHW must complete the Facilitated Enroller Certification Training Program by June 30, 2027.

Requirements

  • Certification as a Community Health Worker or completion of a Community Health Worker training program
  • At least one year of experience as a Community Health Worker or Case Manager in a Community-Based Organization
  • Current New York State LPN license
  • Certification as a Medical Assistant
  • Certification as a Nursing Assistant with proven knowledge of patient care, barriers of care faced by high risk populations, and care coordination of high risk population
  • A degree in the Human Services field
  • Knowledge of patient care, medical terminology, access, and compliance issues (preferred but not required; must be willing to learn)
  • Excellent interpersonal and verbal communication skills
  • Skilled with computers and computer applications
