Erie Family Health Centers is seeking a full time Community Health RN to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills.

Requirements

Current Illinois RN licensure required.
Current BLS for Healthcare Provider CPR certification required.
One to two years’ experience in community health nursing or ambulatory care preferred.

Nice To Haves

Bilingual is a plus.

Responsibilities

Collaborates with providers in the delivery of patient care.
Performs triage assessment process of walk-in patients and telephone calls.
Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
Administers immunizations, applicable contraception refills, and medications as ordered by provider.
Performs other duties as assigned.
