About The Position

The Community Health Worker (CHW) serves as a trusted liaison, connecting the Ohio State Healthy Community Center (HCC) participants from the Near East Side and broader Columbus community to essential resources, while providing guidance and support as they navigate their individualized health and wellness needs. Additionally, the CHW will assist the HCC team in the implementation of nutrition and chronic health education initiatives. Community Health Worker certification is required. The Community Health Worker (CHW) delivers tailored outreach, education, and support services to all people — helping community members navigate healthcare systems, access critical resources, and achieve improvements in health outcomes. The CHW works collaboratively with the Ohio State Healthy Community Center (HCC) team and OSU partners to assist members reach their health and wellness goals. The CHW will also participate in community assessments to identify resources and gaps and assist in the planning and implementation of community health programs. The CHW provides monthly and quarterly reporting and represents the HCC in the community.

Requirements

  • High School graduate or equivalent
  • Community Health Worker certification
  • 1 year of relevant experience required

  • 2-4 years of relevant experience preferred
