About The Position

At IMPaCT Care, the Community Health Program Coordinator serves as the operational backbone supporting Community Health Worker (CHW) teams. The role coordinates program activities, manages data collection and analysis, and helps ensure smooth program operations across multiple sites. The coordinator supports one or two Program Managers overseeing CHW teams serving hundreds of patients and is responsible for enrolling patients in the program. The position is based in Denver, Colorado, and does not allow remote workers. The posting also describes travel to multiple locations across the Atlanta area.

Requirements

Three to five years of experience in program coordination, research administration, or equivalent experience.
Strong writing and communication skills.
Experience with data collection, analysis, and protocol implementation.
Proficiency in Microsoft Word and Excel.
Valid driver's license, a personal car, and car insurance.
Ability to travel to multiple locations across the Atlanta area.
Ability to pass a background check and pre-employment screening.

Nice To Haves

Bachelor's degree preferred.
Bilingual ability, particularly in Spanish or other languages, is a plus.

Responsibilities

Support one or two Program Managers overseeing CHW teams serving hundreds of patients.
Schedule training sessions, team meetings, and community events.
Maintain program documentation, client records, and compliance requirements.
Collect, organize, and analyze data to measure outcomes and improve program quality.
Engage with community members, colleagues, and partners, including people who may be upset.
Enroll patients in the program.
