About The Position

A Community Health Nurse is responsible for coordinating and providing personalized skilled nursing care to patients in their homes. This role involves assessing patients’ health needs, developing and updating individualized care plans, providing skilled clinical services such as wound care, drain management, catheter changes, medication management, and other nursing treatments. In addition, conducting case conferences with a multidisciplinary team that may include therapists, home health aides, dietitians, social workers, nurses, physicians, and other healthcare professionals. The Community Health Nurse monitors patient progress, coordinates services, educates patients and families about medications, treatments, and home safety, connects individuals with community resources, maintains accurate documentation, and ensures that care is delivered safely and effectively. Strong clinical knowledge, communication, organization, and care-coordination skills are essential for promoting patient wellness, independence, and quality of life.

Requirements

  • Graduate from an accredited School of Nursing.
  • Computer and keyboard skills.
  • Medical terminology required.
  • Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing).
  • Current and valid CPR certification.
  • Demonstrates effective interaction and communication (oral, writing, presenting) skills.
  • Ability to work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure.
  • Ability to function competently in stressful situations and a changing work environment related to changing customer needs.
  • Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, etc.).
  • Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations.
  • Ability to use critical thinking and clinical reasoning skills to effectively problem-solve and deliver care.
  • Current and valid driver`s license, registration, inspection sticker and appropriate level of insurance coverage required, to include collision.
  • Employees must adhere to all procedural guidelines in HR Policy 108 including demonstrating an acceptable driving record for the duration of their employment as long as driving continues to be a responsibility of the job.
