About The Position

Registered Nurse (RN) – Home Health – Full Time Fulton County and surrounding area Per visit rates: $50 - $135 Clinical home health / OASIS experience required Offering competitive pay, comprehensive benefits package, referral bonus plan, and mileage reimbursement Bridgeway Home Health provides quality home health care to thousands of individuals in state of Georgia. We are currently growing in Fulton County and surrounding area and looking to add a full-time Registered Nurse to join our clinical, home health field team. Requirements

Graduate of an accredited Registered Nurse educational program
Current Registered Nurse (RN) license within the state of practice
Home Health/Oasis experience required
Current CPR certification
Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
Strong initiative, follow through, process orientation, attention-to-detail
Reliable transportation, valid driver’s license and current automobile insurance

Responsibilities

Conducting one-on-one patient care and patient / family teaching in the home setting.
Perform patient assessments.
Collaborate with the care team to develop and implement a Plan of Care.
Regular communications with our clinical management team.
Timely and accurate documentation.
