Registered Nurse (RN) - Home Health - Full Time
About The Position
Bridgeway Home Health provides quality home health care to thousands of individuals in the state of Georgia. We are currently growing in Forsyth county and surrounding area and looking to add a full-time Registered Nurse to join our clinical, home health field team. Our mission at Bridgeway Home Health is to provide compassionate, home-based care. Requirements
- Graduate of an accredited Registered Nurse educational program.
- Current Registered Nurse (RN) license within the state of practice.
- Home Health/Oasis experience required.
- Current CPR certification.
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills.
- Strong initiative, follow through, process orientation, attention-to-detail.
- Reliable transportation, valid driver’s license and current automobile insurance.
Responsibilities
- Conducting one-on-one patient care and patient/family teaching in the home setting.
- Performing patient assessments.
- Collaborating with the care team to develop and implement a Plan of Care.
- Regular communications with the clinical management team.
- Timely and accurate documentation.
