Community Health Nurse 2 (Local Government)
About The Position
The Delaware County Office of Services for the Aging (COSA) is seeking a motivated and compassionate nurse to join their team as a Community Health Nurse. This role focuses on providing skilled nursing guidance, in-home evaluations, and support for care managers working with older adults in the community. The position includes both office and field responsibilities that contribute to safe, effective services for elders. Requirements
- Two years of community health nursing experience
- A bachelor's degree in nursing, including or supplemented by the required number of credit hours of community health nursing education, accredited by the National League for Nursing
- Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing
- Must meet the PA residency requirement
- Must be able to perform essential job functions
- Satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance
Nice To Haves
- A master's degree in nursing or public health accredited by the National League for Nursing or the American Public Health Association may be substituted for one year of community nursing experience
Responsibilities
- Offer consultation to staff regarding client cases across multiple programs
- Conduct in-home evaluations and determine functional eligibility in various settings
- Identify health or safety hazards in homes and take appropriate corrective steps when feasible
- Participate in multidisciplinary meetings and provide medical input as needed
- Provide training and information to staff, clients, and community members
- Maintain required documentation, data, and reports in accordance with regulations
