About The Position

This role focuses on outreach and engagement with members to enroll them in Reema's care model. The Community Health Navigator will build and manage a caseload of 80-120 members, assessing their needs, including social determinants of health, to create member-centered care plans. The position involves supporting members in understanding health activities, overcoming barriers to care, and coordinating referrals and appointments. Proactive engagement through various channels, including in-person visits and transportation assistance, is key. The role requires maintaining professional boundaries, documenting work thoroughly in the Reema App, and complying with HIPAA standards. The position operates in a fast-paced startup environment, requiring adaptability to change and a commitment to serving members with dignity and respect. Job responsibilities may evolve as business needs change.

Requirements

  • Communication — strong verbal and written skills, including the ability to effectively describe your problem and what you need.
  • Tech Literacy — comfortable learning and troubleshooting new tools on the fly, on both mobile and desktop.
  • Motivation — self-starting enough to stay productive on a remote caseload without regular oversight.
  • Collaboration & Team Building — dedicated to fostering team connection in a remote environment, including participation in occasional required in-person team meetings.
  • 18 years of age or older.
  • Available to work Monday–Friday during core business hours.
  • Use technology-based tools and systems daily.
  • Lift and carry up to 25 lbs occasionally.
  • Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA.

Nice To Haves

  • 1+ years of direct experience with the population/community served, or equivalent.
  • Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent direct experience).
  • Experience with crisis planning, case management, and professional boundaries in community-based work.
  • Relationship-building, community resource knowledge, and health system navigation.
  • Comfort working as part of a multidisciplinary team.
  • Valid driver's license, insurance, and reliable transportation [If field-based].
  • Pre-employment drug screening and background check.

Responsibilities

  • Conduct phone and in-person outreach and enroll members in Reema's care model.
  • Build and manage a caseload (80-120 members), providing coverage for other Guides during absences or workload spikes.
  • Assess member needs, including social determinants of health, to build a member-centered care plan.
  • Establish trusting relationships and reliably follow through in a timely, appropriate manner.
  • Help members understand why a health activity matters, work through hesitation with empathy, and follow through to completion.
  • Help members steadily overcome barriers to care, with visible progress every month before the program ends.
  • Coordinate referrals and support appointment scheduling and attendance.
  • Independently organize and prioritize your workload to hit monthly goals and daily metrics.
  • Proactively engage with members using a comprehensive mix of outreach channels, including phone, text, email, and in-person visits.
  • Connect members to community and health resources, including transporting or accompanying them as needed.
  • Maintain professional boundaries and confidentiality; consult the multidisciplinary team on complex situations.
  • Document all member work thoroughly and on time in the Reema App, including keeping care plans current.
  • Comply with HIPAA and protected health information confidentiality standards.
  • Work independently and collaboratively in a fast-paced startup — see frequent change as an opportunity, not a barrier.
  • Serve members with dignity and respect; build and maintain relationships with community resources.
