As a Community Health Coordinator, you will support Care Management by coordinating Community Health Worker (CHW) visit schedules and producing accurate schedules, reports, and documentation for Supervisor use. Key responsibilities include scheduling home/field appointments, calling members to arrange CHW visits with a Service Excellence mindset, maintaining schedules in Outlook and Excel, entering staff/client data in databases, tracking statistics and reports, and completing general office support (documents, filing, shredding, mail, reception/phones). You will also identify improvement opportunities and escalate issues as needed.
