About The Position

VillageCare is a community-based, not-for-profit organization serving people with chronic care needs, as well as seniors and individuals in need of continuing care and managed care services. Our mission is to promote healing, better health and well-being to the fullest extent possible. Our care is offered through a comprehensive array of community and residential programs, as well as managed care. VillageCare has delivered quality health care services to individuals residing within New York City for over 45 years. As a Community Health Coordinator, you will support Care Management by coordinating Community Health Worker (CHW) visit schedules and producing accurate schedules, reports, and documentation for Supervisor use. Key responsibilities include scheduling home/field appointments, calling members to arrange CHW visits with a Service Excellence mindset, maintaining schedules in Outlook and Excel, entering staff/client data in databases, tracking statistics and reports, and completing general office support (documents, filing, shredding, mail, reception/phones). You will also identify improvement opportunities and escalate issues as needed.

Requirements

  • High School diploma required
  • Valid NYS ID or NYS Driver License
  • Proficiency in Microsoft Word, Outlook, and Excel
  • Strong written and verbal communication skills

Nice To Haves

  • Associate's degree preferred
  • 2-3 years healthcare experience preferred
  • Bilingual preferred (Spanish, Russian, Chinese, Bengali, Creole)
  • Organized, detail-focused, and service-oriented
