The Fund for Public Health in New York City (FPHNYC) is seeking a Community Health Worker to provide point-of-care caregiver and family education for pediatric asthma self-management and support. This role is part of the Center for Health Equity & Community Wellness (CHECW), which aims to eliminate racial and other inequities that lead to premature mortality. CHECW works to address systemic inequities across community and healthcare systems by focusing on social determinants of health, including environmental and commercial factors. This position is grant-funded and will focus on engaging pediatric patients, caregivers, and families to assess asthma exacerbation causes, provide education on asthma management, and connect them with relevant resources.
