About The Position

This recruitment will establish an open eligible list and will be used to fill current and future Community Health Assistant II vacancies within the Health Care Agency. This recruitment may also be used to fill positions in similar and/or lower classifications throughout the County of Orange. The County of Orange, Health Care Agency (HCA) is a dynamic organization dedicated to creating and supporting an environment that promotes the achievement of optimal individual, family and community health. Public Health Services is a multidisciplinary service area composed of six synchronized divisions—Clinical Services, Communicable Disease Control, Community and Nursing Services, Health Promotion, Environmental Health, and the OC Public Health Laboratory. PHS engages with an array of OC Health Care Agency (HCA) services, and in collaboration with community partners, in the pursuit of preventing disease and creating conditions that enhance the health of individuals and communities. The Pulmonary Disease Services (PDS) Tuberculosis (TB) Program provides diagnosis, treatment and case management for individuals with active or suspected TB disease. The Clinic evaluates patients, performs case management and administers treatment for those with active or suspected TB disease. It also provides screening services and treatment for latent TB infection among individuals at the highest risk of progression to active TB disease. The Community Health Assistant II (CHA II) position is with the Clinical Services Division, Pulmonary Disease Services (PDS) Tuberculosis (TB) Program. To ensure adherence to the TB treatment regimen lasting at least six-months, patients are placed on Directly Observed Therapy (DOT). The primary responsibility of the CHA II position is to provide DOT services (in-person and video) to TB patients. The position is critical to TB control to help ensure that patients complete treatment, reduce TB transmission in the community and prevent the development of drug resistance.

Requirements

• Two (2) years of experience with interviewing, counseling, and advising others on a health subject specialized to different programs related to healthcare.
• Understanding and application of Health Insurance Portability and Accountability Act (HIPAA)
• Software proficiency with Microsoft Word, Excel, PowerPoint and Outlook
• Implementing best practices including, but not limited to, delivering client/patient-centered care and performing comprehensive services
• Knowledge of State mandates, laws and County regulations related to Public Health and communicable disease control
• Experience with the use of an Electronic Health Record
• Establishing trust and creating a positive rapport with participants, partners, and the community
• Establishing and cultivating effective working relationships with various staff members, community agencies, and resources
• Participating in projects as a member of the team and working on assigned tasks independently
• Communicating effectively and professionally both verbally and in writing with individuals who may be facing medical, socioeconomic challenges, as well as external and internal partners within the Agency
• Communicating with tact and discretion when working with confidential information
• Presenting information that actively engages the audience in clinics and community centers
• Providing health education and information that is culturally appropriate to various communities of socioeconomic and cultural backgrounds
