About The Position

ViaQuest Psychiatric & Behavioral Solutions provides individualized care to children, adolescents, adults, older adults, people with developmental disabilities, and people experiencing extreme trauma. Clients are supported by a multidisciplinary team that includes mental health counselors, social workers, certified nurse practitioners, case managers, and psychiatrists. The Community Based Therapist provides therapy and counseling services, including to clients in community and day program settings, and collaborates with other members of the care team. The position is full time and involves travel throughout an assigned service area.

Requirements

Bachelor's degree in Social Work or a related field and one year of therapy experience, or a Master's degree in Social Work or Professional Counseling.
A Social Work or Professional Counseling license or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, LMFT, LIMFT, or LIMFT-S.
Valid driver's license and reliable transportation.
Willingness to travel throughout the assigned service area.

Responsibilities

Perform comprehensive diagnostic assessments.
Provide patient visits as scheduled.
Keep each client's treatment plan up to date.
Complete comprehensive and succinct progress notes.
Communicate and follow up as needed regarding client status.
Collaborate with professionals providing case management and/or psychiatric services.
