Community Based Therapist
About The Position
ViaQuest Psychiatric & Behavioral Solutions provides individualized care to children, adolescents, adults, older adults, people with developmental disabilities, and people who have experienced extreme trauma. Its multidisciplinary teams include mental health counselors, social workers, certified nurse practitioners, case managers, and psychiatrists. The Community-Based/Day Program Therapist provides therapy and counseling services, including comprehensive diagnostic assessments and scheduled patient visits. The therapist maintains current treatment plans and concise progress notes, communicates and follows up on clients’ status, and collaborates with professionals providing case management and psychiatric services. The position requires travel throughout an assigned service area and offers flexible scheduling.
Requirements
- Bachelor's degree in Social Work or a related field with one year of therapy experience, or a Master's degree in Social Work or Professional Counseling.
- Social Work/Professional Counseling license or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, LMFT, LIMFT, or LIMFT-S.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout the assigned service area.

Responsibilities
- Perform comprehensive diagnostic assessments.
- Provide patient visits as scheduled.
- Ensure treatment plans are up to date for each client.
- Complete comprehensive and succinct progress notes.
- Communicate and follow up as needed regarding the client's status.
- Work collaboratively with those providing case management and/or psychiatric services.
