About The Position

The Resource Navigator will support the implementation of the Attleboro Early Access & Family Stabilization Initiative by increasing access to health, behavioral health, and social supports for students, families, and staff. This role focuses on proactive outreach, coordinated referral support, and reducing barriers to care, particularly for underserved populations including multilingual families, financially insecure households, and families experiencing housing instability. This is a 24 month, grant funded position.

Requirements

  • Bachelor's degree in social work, human services, education, communications, community development, or a related field required
  • Bilingual/Multilingual
  • Experience working with families, schools, and community-based organizations
  • Familiarity with local health, behavioral health, and social service systems
  • Ability to work collaboratively and effectively communicate with school staff, families, and community partners
  • Experience working with diverse populations
  • Ability to navigate systems and support families in accessing services
  • Valid driver’s license required

Nice To Haves

  • Master's degree preferred
  • Strong data analysis and technical planning skills

Responsibilities

  • Increase access to behavioral health services, primary care, insurance navigation, housing, food, and other social service resources by identifying and mitigating barriers faced by APS students, families, and staff.
  • Conduct proactive outreach to underserved populations through school events, community partnerships, and targeted engagement efforts
  • Support the review, refinement, and implementation of referral navigation and data tracking systems to improve efficiency and equitable access to services.
  • Support staff and families in navigating referral processes across school and community-based systems by assisting with communication, follow-up, and connection to appropriate services and providers
  • Participate in regular meetings with school staff, Community Resource & Support Liaisons, and community organizations to strengthen referral pathways and coordinated care across systems
  • Maintain and regularly update the APS Community Resource Guide and Community Flyers page to ensure families and staff have access to current information and resources
