About The Position

HCR is offering a one-day CNA Transition Class to obtain a Home Health Aide (HHA) Certificate. This NYS Department of Health approved course will enable participants to obtain a recognized HHA Certificate and work for HCR serving home care patients in Clinton County. Work is readily available.

Requirements

• Active CNA Certificate
• High School diploma or GED preferred
• Successful completion of HCR's Reading and Math Test
• Reliable transportation
• Driver's License
• After offer acceptance: Provide a copy of immunization records/MMRs
• After offer acceptance: Submission to CHRC background check, fingerprinting, drug test, physical exam and Tuberculin Testing

Responsibilities

• Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.
• Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
• Immediately report changes in client’s condition to supervising professional.
• Ensure the safety of the patient, patient’s family and/or the employee.
• Maintain HIPAA.
