About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to comfortably and safely perform duties in these environments.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.

Multi-state licenses.

Responsibilities

Conduct In-Home Health Evaluations.
Connect one-on-one with individuals to answer health questions.
Perform medication and medical history reviews.
Conduct physical evaluations.
Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
