About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct approximately 45-minute visits, including medication and medical history review, physical evaluation, and potentially diagnostic tests. The role utilizes an iPad with a clinical workflow and does not require prescribing medication, ordering labs, or altering current treatment plans. The clinician will be part of a national network focused on improving health outcomes and connecting people to the right care.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
• Must be able to travel locally and within licensed states, as needed.
• Must be able to conduct virtual visits as needed.

Responsibilities

• Conduct In-Home Health Evaluations.

• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
