About The Position

Frontier Behavioral Health's Trueblood Housing - Permanent Supportive Housing Team serves Trueblood class members, who are individuals with behavioral health conditions involved in the criminal justice system and residing in permanent supportive housing. This role is crucial for maintaining a safe, stable, and trauma-informed environment that fosters recovery, dignity, and long-term housing success. The team builds trusting relationships with residents, provides emotional and practical support, assists with daily living activities, and helps residents access services like behavioral health, legal aid, and employment programs. Responsibilities include documenting interactions, collaborating with case managers and healthcare providers, and upholding trauma-informed care, harm reduction, and housing first principles. The Clinician I is part of a multi-disciplinary team providing outreach and engagement services to individuals with a history in the forensic mental health system, working closely with other Trueblood Settlement Programs to provide case management and housing support. Services provided by this team will not duplicate those offered by other health and behavioral health programs but will leverage existing services to enhance support for enrolled individuals.

Requirements

  • Bachelor's degree in psychology, counseling, social work or related field from accredited college or university.
  • Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.
  • Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.

Nice To Haves

  • Two years of experience working with psychiatrically impaired populations.
  • Experience in the criminal justice system.
  • Experience working in housing programs.
  • Experience creating and conducting educational and/or skill building groups.

Responsibilities

  • Maintain a safe and welcoming environment for residents through regular monitoring and de-escalation support.
  • Build trusting relationships with residents and provide emotional and practical support.
  • Assist residents with daily living activities, including hygiene, meals, and medication reminders.
  • Support residents in accessing services such as behavioral health, legal aid, and employment programs.
  • Document incidents, resident interactions, and service coordination in compliance with program standards.
  • Collaborate with case managers, peer support specialists, and health care providers.
  • Uphold trauma-informed care, harm reduction, and housing first principles.
  • Locate individuals identified on the HCA referral list.
  • Create or further develop recovery plans and on-going support to promote progress in care.
  • Provide crisis and hospital diversion services to individuals at high risk.
  • Complete suicide and homicide risk assessments to determine a level of response needed to maintain the safety of the client or others.
  • Assist non-Medicaid individuals to access Medicaid as appropriate.
  • Assist with obtaining SOAR services.
  • Provide evaluation and treatment for patients who have behavioral health concerns.
  • Assist and complete care plans and provide assessments and referrals for outpatient or inpatient treatment.
  • Provide consultation, training, and education to staff regarding behavioral health issues as assigned by supervisor.
  • Facilitate use of/access to crisis hospital diversion resources, including Stabilization and/or E&T admission.
  • Identify and include the client’s support network in care coordination.
  • Assist clients in identifying and accessing resources which will meet their needs.
  • Coordinate and collaborate with community resources on behalf of clients.
  • Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
  • Travel to see clients in the community and travel between work sites.
  • Other duties as assigned.
