Position Title: Permanent Housing Care Coordinator Employment Classification: Full-Time; Hourly; Non-Exempt Shift: Monday-Friday 9:00 AM – 5:30 PM Reports to: Cal-AIM Housing Supervisor Department: Program Program Summary: This position will be working primarily with TAY families, with a TAY (18-25) head of household, who meet the criteria for this project-based voucher site. This position also includes working with young adults exiting any Orangewood housing program and supporting their permanency through monthly, and as needed contacts, with the Housing Aftercare. Position Summary: The Permanent Housing Care Coordinator is responsible for working with TAY families at the Richard Lehn Intergenerational Campus, located in Santa Ana. This position also includes providing Care Coordination with young adults exiting any of Orangewood’s housing programs and supporting their permanency in the community through the Permanent Supportive Housing Care Coordination model as well as through the Housing Aftercare Program.
