About The Position

This position is responsible for performing a variety of duties involving patient assessment, psychotherapy, and coordination of services. The Clinical Triage and Intake Coordinator collaborates as a member of an inter-disciplinary treatment team for the development and delivery of treatment plans and discharge plans for patients. They are a frontline healthcare professional responsible for receiving new patient referrals, conducting initial clinical assessments, and determining the appropriate level of care. This role verifies insurance, manages Electronic Health Records (EHR), and coordinates with providers to ensure prompt, accurate patient placement and care. Key responsibilities include clinical assessment and triage, communication and liaising with patients, families, and referral sources, managing waitlists and the admission process, performing initial diagnostic evaluations, documenting assessments in the EHR, and collaborating with medical staff on care plans. The role also involves community outreach and education.

Requirements

  • Masters degree in Social Work.
  • Current Massachusetts Licensed Clinical Social Worker (LICSW) licensure.
  • One to three years of social work experience
  • Demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, ability in discharge planning, and ability to work with a multidisciplinary team.
  • Data compilation and analysis skills.
  • Excellent written and communications skills.
  • Basic computer skills.

Responsibilities

  • Conducts intake interviews with patients and families to develop psychosocial evaluations.
  • Participates in team planning of treatment during initial interview and coordinates team meeting on a daily basis.
  • Provides psycho-social assessments and treatments, individual and group education/therapy to include dual diagnosis cases and all associated social work services.
  • Serves as a primary clinician for individual therapy, couples and family therapy.
  • Works with outpatient providers to coordinate treatment planning.
  • Documents all information obtained in evaluation, assessment, consultation, referral and disposition to comply with regulatory requirements, as well as ensuring best possible patient outcome.
  • Coordinates discharge planning which includes the assessment of housing, benefits and treatment needs. Plays key role in case management.
  • Acts as liaison with community and government agencies and develop referral network as directed.
  • Complies with established departmental policies, procedures and objectives.
  • Attends variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.

  • Maintains, regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.
  • Evaluates patient symptoms and medical history to determine urgency and appropriate care, such as directing mobile crises, PHP/IOP, interventional psychiatry, medication management, outpatient therapy for individual and/or therapeutic groups.
  • Acts as a primary contact for patients, families, and referral sources to foster positive relationships and ensure smooth, and compliant patient onboarding. Managing inquiries via fax, phone, email, and EHR.
  • Manages waitlists and the full admission process, including gathering patient information, verifying insurance coverage, obtaining necessary authorizations, confirming patient financial responsibility and scheduling initial behavioral health appointments.
  • Perform initial diagnostic evaluation and formulate preliminary treatment plans, as needed.
  • Accurately records assessments, patient history, legal status, and insurance information within the Electronic Health Record (EHR) system. Ensuring all forms are accurate, signed, and maintained in compliance with HIPAA regulations.
  • Collaborates with medical staff to set patient goals and coordinate appropriate behavioral health programming.
  • Facilitates education on programming and referral process.
