About The Position

The Clinical Therapist provides mental health therapy services to children, youth, and adults with Intellectual/Developmental Disabilities (IDD) in an integrated health clinic. The role supports case managers in formulating individualized treatment plans used with the primary care team to develop holistic care strategies for clients. The position offers a hybrid work schedule, contingent on meeting productivity and telehealth visit standards established by leadership. Employees may work remotely for part of their schedule, but must prioritize client needs and provide in-person appointments in the office or community when requested. Staff are required to work in the office during their first 30 days; this period may be extended based on performance. The hybrid schedule may be adjusted at any time based on agency needs.

Requirements

Master of Social Work (MSW).
Required licensure: LMSW, LMFT, LPC, or LLMSW.

Responsibilities

Prioritize work assignments according to consumer and program needs while using available work time efficiently.
Provide psychotherapeutic support and counseling to clients according to DSM-5 guidelines.
Discuss continuing care needs with the multidisciplinary care team.
Ensure proper handling of confidential medical records in the electronic medical record (EMR).
Maintain requests for medical records from other sources.
Plan follow-up for clients with post-acute transitional concerns.
Document assessment findings in SW outpatient assessments or SW progress notes in the EMR.
Communicate material findings and tentative transition plans to the care team, including nursing, physicians, and case management.
Complete clinical and statistical records of staff activities accurately and on time.
Document every consumer, guardian, and collateral contact in compliance with agency policy.
Complete administrative forms to support accurate data collection for billing, fee setting, the management information system, and other purposes in accordance with agency policy.
