About The Position

Review, assess, and manage medication orders, including, but not limited to, oral oncology therapies, to identify, prevent, and resolve actual or potential drug-related problems. Provide patient and provider education, drug information, and counseling; make clinical recommendations and communicate with physicians and other clinicians via phone and EMR. Recommend appropriate formulary alternatives and ensure accurate compounding, dispensing, and delivery of medications in accordance with safety and pharmacy standards. Oversee and support pharmacy technicians involved in medication preparation, maintenance, and dispensing; ensure efficient workflow within the specialty pharmacy department. Support academic and precepting responsibilities for pharmacy students and float as needed to provide pharmacy coverage and maintain continuity of patient care.

Requirements

Pharmacist obtained prior to hire date or job transfer date required.
Licensure required relevant to state in which work is performed.
Bachelor's or advanced degree in Pharmacy required.
Embedded Pharmacy experience.
Specialty Pharmacy experience.
