Requirements

A Master's degree in social work, psychology, or another human services field
At least two years of post‑bachelor's experience working with individuals with severe and persistent mental illness
Strong knowledge of community resources and support agencies
A valid New Jersey driver's license with a clean five‑year driving record
A flexible schedule and a commitment to meeting client needs with empathy and professionalism

Spanish-speaking ability strongly preferred

Responsibilities

Participating in daily meetings, case discussions, and treatment planning as part of a collaborative PACT team.
Providing individual and group counseling using evidence-based practices.
Conducting emergency mental health assessments.
Coordinating closely with medical staff regarding client medication needs.
Supporting families and natural support systems in understanding and engaging in the treatment process.
Assisting with transportation needs to help clients move toward greater independence.
Participating in the team's on-call rotation.
Contributing to continuous quality improvement efforts.
