About The Position

Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.

Requirements

  • Bachelor's Degree or equivalent related field
  • 2 years of relevant related experience
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • Or LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required
  • Or CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration Upon Hire required
  • Or CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants Upon Hire required
  • Or LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan or equivalent in mental health/behavioral health Upon Hire preferred
  • Or LIC-Physical Therapist - STATE_MI State of Michigan Upon Hire required
  • Or CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire required
  • Or CRT-Speech Language Pathologist - ASHA American Speech-Language-Hearing Association Upon Hire required
  • Or LIC-Respiratory Therapist - STATE_MI State of Michigan Upon Hire required
  • Or CRT-Athletic Trainer, Certified - BOC Board of Certification and Accreditation Upon Hire required
