This role supports a large Primary Care Practice in Grand Haven, Michigan, focusing on Internal Medicine and Family Medicine. The Clinical Pharmacy Specialist will collaborate closely with physicians and advanced practice providers, working under a Collaborative Practice Agreement (CPA) to optimize medication regimens for patients. Responsibilities include providing direct patient care for chronic disease and medication therapy management, assisting patients with medication-related barriers, educating clinic staff on medication updates, and participating in team-based care planning. The specialist will utilize the EMR (EPIC) daily, manage chronic disease states, and build strong relationships with providers and staff. They will also be involved in monitoring and improving system quality measures, navigating formularies to reduce costs, and precepting pharmacy residents and students. The role requires evaluating patients, interpreting drug serum levels, and providing dosage recommendations to enhance therapeutic outcomes and reduce costs. Additionally, the specialist will serve as a drug information resource and role model for staff pharmacists, coordinate drug utilization reviews, and engage in process and quality improvement activities. Continuous professional development and completion of required training are essential. The specialist will also coordinate the development and dissemination of departmental publications.
