The pharmacist in this clinic will support a group of 10 Physicians practicing in Internal Medicine or Family Medicine. These Physicians are supported by a team of 8 Advanced Practice Providers, 2 Nurse Care Managers, 2 Social Workers, 2 Clinic Nurses, and 1 Pharmacist. The Clinical Pharmacy Specialist will work closely with providers under a Collaborative Practice Agreement (CPA) to evaluate and optimize medication regimens for mutual patients. They will provide guideline and evidence-based Chronic Disease and Medication Therapy Management directly to patients within a clinic setting, assist patients with cost, adherence, and lifestyle barriers related to their medications, and work closely with clinic staff to inform them of guideline and medication updates. Collaboration with other care team members to develop care plans and daily navigation of EPIC and utilization of EMR features are key aspects of this role. The specialist will also be responsible for chronic disease state management and/or medication therapy management directly to patients within the clinic setting, building relationships with providers and staff, and following Corewell Health’s standard work, policies, and procedures to gather and interpret patient vitals and labs during visits. They will locate and use clinical data within the EMR to assist with developing future treatment plans, participate in team-based approaches to determine the best treatment plan for the patient, and communicate within the clinic care team to determine which patients would benefit the most from Pharmacy services. Regular collaboration with providers and office staff will occur to facilitate educational updates on guidelines, new medications or therapies, and best practices, and to keep the clinic updated on any medication-related changes. The role also involves assisting the clinic in monitoring and improving upon system Quality Measures, helping patients and clinic staff navigate formularies and other programs to assist in lowering the cost of care, and precepting pharmacy residents and students independently. The specialist will evaluate patients and orders, take a lead role in ordering and interpreting drug serum levels using pharmacokinetic principles, and provide physicians with dosage regimen recommendations. They will also provide drug information and educational services to healthcare professionals, patients, and families, serve as an information resource and clinical practice role model for staff pharmacists, coordinate and implement departmental drug utilization review efforts and other process improvement activities, and ensure processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engagement in process and quality improvement activities, making and implementing recommendations to improve operational efficiency and implement new services, and facilitating applicable Pharmacy Committee activities are also part of the responsibilities. Continuous study of the professional field to maintain competence and completion of required continuous training and education are expected.
