About The Position

Under the direction of the Clinical Manager, this role ensures optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. The specialist provides drug information to patients and families, physicians, nurses, and other healthcare professionals. They are integrally involved in the education of pharmacists, residents, and students, serving as a resource and drug expert in infectious disease treatment and prevention. The role also involves research and writing activities to advance knowledge and may coordinate the acquisition, distribution, and control of drug therapy for patients as needed.

Requirements

  • Licensure to practice pharmacy in the state of Massachusetts by the hire date.
  • Certification (such as BPS or accepted by CCP, required before or within a reasonable time after onboarding).
  • Substantial interpersonal skills towards customers (patients, physicians, nurses) and fellow employees.
  • Deep knowledge of medications, their effects, drug interactions, medical procedures and therapies, diagnosis, and disease states.
  • Effective interpersonal skills to interact appropriately with hospital staff, patients, families/visitors, physicians, colleagues, and others.
  • Ability to multi-task, prioritize essential tasks, and meet deadlines.
  • An organized work style and ability to keep well-written documentation.
  • Excellent verbal and written communication skills.
  • Must possess self-management skills and require little to no supervision.
  • Must be a self-starter, self-motivating, have a strong work ethic, and the ability to identify issues or opportunities that require attention and change.
  • Must possess and exhibit process improvement skills and acumen.
  • Strong attention to detail and accuracy.
  • Experience with Microsoft programs (MS Word, Excel, Outlook) and web browsers.

Responsibilities

  • Evaluation and management of chronic conditions (HIV, other infectious diseases, PrEP, Hepatitis C) through direct patient care under a collaborative practice agreement in an ambulatory clinic.
  • Teaching and precepting APPE 4th year PharmD students, PGY-1 and PGY-2 practice residents.
  • Collaboration with interdisciplinary healthcare professionals to improve and implement transitions of care (TCM) for hospitalized patients.
  • Develop and assist with the BMC HIV specialty drug management program.
  • Participate in Clinic/Pharmacy department related quality improvement initiatives, outcomes research, and provider education.
  • Actively participate in ambulatory formulary review process and various institutional committees.
  • Serve as a resource for medication management and care coordination for patients in the BMC Specialty Care Management program.
  • Collaborate on institutional delivery system transformation initiative projects focused on improving population health outcomes.
  • Performs other duties as needed.
