The Infectious Disease, Pulmonary Hypertension, & Neurology Ambulatory Clinical Pharmacist position offers a unique opportunity to provide advanced pharmacy services across multiple highly specialized clinics at National Jewish Health. This role supports patients with complex infectious, neurologic, and cardiopulmonary conditions requiring high-touch care coordination, specialized therapies, and close clinical monitoring. Working collaboratively with multidisciplinary teams, the pharmacist will play a key role in optimizing therapy outcomes, improving medication adherence, and enhancing patient access to life-changing treatments. At National Jewish Health, pharmacists are integral members of multidisciplinary care teams dedicated to innovation, collaboration, and excellence in patient-centered care. In this role, the Infectious Disease, Pulmonary Hypertension, & Neurology Ambulatory Clinical Pharmacist will help advance specialized care for patients with complex chronic conditions while contributing to improved outcomes and quality of life. The Ambulatory Clinical Pharmacist will serve as an integral member of the interdisciplinary care team, working collaboratively with providers, advanced practice providers, nursing staff, and other healthcare professionals to optimize medication therapy for patients with chronic conditions. This pharmacist will focus on guideline-directed therapy, chronic disease management, medication optimization, and patient education through a team-based care model.
