About The Position

Under the direction of the Clinical Manager, ensures optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Provides drug information to patients and families, physicians, nurses, and other healthcare professional. Integrally involved in education of pharmacists, residents and students. Serves as a resource and drug expert in the infectious disease treatment and prevention specialty areas. Strives to increase knowledge through research and writing activities. The Clinical Specialist may also be involved, as necessary, in coordinating the acquisition, distribution and control of drug therapy for patients under the care of the pharmacist.

Requirements

  • Licensure to practice pharmacy in the state of Massachusetts by the hire date.
  • Substantial interpersonal skills towards our customers (particularly patients, physicians and nurses) and fellow employees of the hospital.
  • Deep knowledge of medications, their effects, drug interactions, medical procedures and therapies, diagnosis and disease states required.
  • Effective interpersonal skills to interact appropriately with hospital staff, patients, families/visitors, physicians, colleagues, and others.
  • Ability to multi-task, prioritize essential tasks and meet deadlines
  • An organized work style and ability to keep well written documentation.
  • Excellent verbal and written communication skills.
  • Must possess self-management skills and require little to no supervision.
  • This person must be a self-starter, self-motivating, have a strong work ethic and must have the ability to identify issues or opportunities that require attention and change.
  • Must possess and exhibit process improvement skills and acumen.
  • Strong attention to detail and accuracy.
  • Experience with Microsoft programs (i.e. MS Word, Excel, Outlook) and web browsers

Nice To Haves

  • Certification (such as BPS or accepted by CCP, required before or within a reasonable time after onboarding)
  • Additional language skills can be a plus.

Responsibilities

  • Evaluation and management of chronic conditions that may include HIV, other infectious diseases managed in an ambulatory setting, PrEP, Hepatitis C, etc. through direct patient care working under a collaborative practice agreement within an ambulatory clinic.
  • Teaching and precepting APPE 4th year PharmD students, PGY-1 and PGY-2 practice residents
  • Collaboration with a team of interdisciplinary healthcare professionals to improve and implement transitions of care (TCM) program for hospitalized patients.
  • Develop and assist with BMC HIV specialty drug management program
  • Participate in Clinic/Pharmacy department related quality improvement initiatives, outcomes research and provider education
  • Actively participate in ambulatory formulary review process and various institutional committees
  • Serve as a resource for medication management and care coordination for patients enrolled in BMC Specialty Care Management program
  • Collaborate on active institutional delivery system transformation initiative projects focused on improving population health outcomes.
  • Performs other duties as needed.
