The Clinical Pharmacy Coordinator - Medication Safety is responsible for demonstrating expertise in their area of practice, providing clinical support to colleagues, and solving complex problems. This role supports department operations in compliance with all regulations and policies. The coordinator participates collegially with pharmacy and healthcare teammates to ensure pharmacy representation in medication use management programs, including clinical guideline development and drug use programs. They serve as a clinical pharmacy liaison, applying expertise in drug therapy to prevent and resolve drug-related problems. A key aspect of this role is designing, educating, evaluating, and leading safety and performance improvement initiatives, including developing educational programs for medical, nursing, and pharmacy teammates to achieve clinical quality, cost, and service goals. The coordinator also identifies opportunities for stewardship of high-cost or high-use medications, executes action plans, and tracks stewardship initiatives. Additionally, they mentor, orient, train, and assist in performance evaluations of pharmacy teammates, residents, and students, and support team leaders in executing initiatives. The role may also involve performing duties of relevant pharmacy teams, such as order verification, medication preparation, patient counseling, medication management, transitions of care support, and utilization management reviews. Pharmacists under a collaborative practice agreement may need to demonstrate competence in chronic disease management. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the life span and assessing patient status based on age-specific needs.
