The Clinical Pharmacist provides leadership within a specialized healthcare team to support optimal, cost-effective drug therapy. The role includes developing individualized pharmacotherapeutic goals, recommending treatment regimens, and creating monitoring plans. The pharmacist also leads efforts to prevent and report medication errors and adverse drug reactions and to continually improve medication safety and practice. This onsite position rotates among One Hundred Oaks, Main Campus, and Melrose Support Services. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with no weekends or holidays. The pharmacist independently manages daily responsibilities with substantial autonomy, initiative, and accountability, bridging outpatient clinic care and pharmacy services so patients can access prescribed medications in a timely manner. Duties include supporting medication access and affordability, responding to medication-related questions, and fostering communication and collaboration across care teams. The pharmacist reviews prescriptions and drug orders, prepares and dispenses medications, provides drug information, manages medication therapy, and supervises support personnel. The role requires maintaining high standards of clinical pharmacy quality, safety, and regulatory compliance. A Tennessee pharmacist license and a bachelor's degree are required, along with one year of relevant work experience. Retail or community pharmacist experience is highly preferred; management experience is preferred but not required.
