About The Position

Sentara is hiring a Flexible Time Pharmacist for our Central Medication Refill Program. This remote, PRN/as needed position is for the Day shift. The Pharmacist manages patient drug therapy in accordance with applicable clinical practice standards and assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety. The role is responsible for providing safe, effective, and timely pharmacy services while supporting optimal patient outcomes through evidence-based medication management. They must maintain current knowledge of medications, disease states, treatment guidelines, and emerging best practices relevant to assigned areas of practice. A valid Pharmacist (RPh) license in the state of practice is required.

Requirements

  • Must possess and maintain a valid Pharmacist (RPh) license in the state of practice.
  • Bachelor's Level Degree in Pharmacy (Required)
  • Virginia Pharmacist License (Required)
  • Basic Life Support (BLS) (Required)

Nice To Haves

  • Doctorate Level Degree in Pharmacy (Preferred)
  • 2 years' experience in Clinical or Ambulatory Pharmacy (Preferred)

Responsibilities

  • Independently performs pharmacist responsibilities by exercising sound clinical judgment when reviewing, evaluating, and managing medication-related requests to ensure safe and effective patient care.
  • Maintains current knowledge of medications, disease states, treatment guidelines, and emerging best practices to support appropriate, evidence-based medication management and compliance with applicable requirements.
  • Serves as a trusted clinical resource for medication-related information, demonstrating strong customer service, communication, problem-solving, and decision-making skills while fostering collaborative relationships to support coordinated patient care in a primarily electronic workflow environment.
  • Supports quality assurance, performance improvement, and patient safety initiatives, ensuring compliance with departmental, organizational, and regulatory standards while identifying and escalating concerns outside established scope or protocols as appropriate.
  • Adheres to organizational policies, procedures, regulatory requirements, and professional standards.
  • Utilizes electronic health records, pharmacy systems, and other technology platforms accurately and efficiently.
  • Demonstrates ownership and accountability for assigned responsibilities, projects, and operational goals while contributing to process improvement and departmental initiatives.
  • Serves as a role model for professionalism, collaboration, teamwork, and continuous improvement.
  • Participates in onboarding, training, mentoring, and education of team members, students, and other learners as assigned.
  • Precepts pharmacy students, residents, and other learners as assigned.
  • Performs other duties and responsibilities as assigned.
