About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist to join their Pharmacy Clinical Operations 2 team. This role provides leadership within a specialized healthcare team to ensure optimal, cost-effective drug therapy. Responsibilities include developing pharmacotherapeutic goals, recommending treatment regimens, and creating monitoring plans tailored to individual patient needs. The clinical pharmacist is also responsible for leading efforts in preventing and reporting medication errors and adverse drug reactions, and continuously improving processes to enhance medication therapy safety. This position offers a 7 on/7 off schedule, working overnight hours (7:00 PM - 7:00 AM), primarily supporting patients in various ICUs and the emergency department. Additional responsibilities include consult coverage, addressing clinical questions, and providing code response for adult inpatients. Opportunities for engagement with other shifts, projects, precepting, and committees are available based on individual interests. VUMC is looking for motivated individuals to help expand clinical scope and practice.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year of experience
  • Bachelor's degree (Required)

Nice To Haves

  • PGY2 in ICU or emergency medicine or equivalent experience.
  • PGY1 plus experience in critical care setting.

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
  • Develops specific pharmacotherapeutic goals.
  • Recommends a regimen and develops a monitoring plan to meet the individualized drug therapy needs of the patient.
  • Leads efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy.
  • Works to continuously improve process and practice in the institution to enhance safety of medication therapy.
  • Provides consult coverage, clinical questions, and code response for admitted patients in the adult hospital.
  • Mentoring & Coaching (Intermediate): Demonstrates good training and mentoring practices on a regular basis. Acts as a mentor to a person in need of support and direction.
  • Medication Management (Advanced): Demonstrates the uppermost levels of expertise in medication management in challenging and complex situations.
  • Pharmaceutical Drug Preparation & Dispensing (Novice): Possesses sufficient fundamental proficiency to successfully review, prepare and dispense prescribed medications.
  • Pharmaceutical Quality Assurance & Compliance (Expert): Demonstrates expert knowledge of the appropriate rules and regulations as they apply to the organization.
  • Pharmaceutical Computer Literacy (Novice): Has demonstrated a proficient understanding of computers or has successfully completed training for applications that are used to support medication use processes.
