About The Position

Axil Health is seeking a dedicated Clinical Pharmacist to lead their Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities across North Carolina. The Clinical Pharmacist will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice. The role is performed in provider clinics and may require regional travel to a group of clinics, requiring the candidate to live within a commutable distance to the posted job location.

Requirements

  • A current, valid pharmacist license to practice in North Carolina.
  • Current CPR certification.
  • Certification for Immunization.

Nice To Haves

  • PharmD Degree with three years of experience.
  • Currently, hold a Board of Pharmaceutical Specialty Certification or Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
  • Completion of an APhA Certificate Training Program.

Responsibilities

  • Build and work directly with your patient panel to drive clinical outcomes and results.
  • Spearhead patient enrollment, device management, and data interpretation for RPM.
  • Collaborate on medication management with supervising providers.
  • Participate and lead topic discussions for current staff and residents.
  • Precept PGY-1 Students and participate in our Fellowship Program.
  • Develop individualize care plans based on specific patient needs.
  • Provide recommendations or implement therapy changes, when necessary.
