About The Position

Baptist Health is seeking a Decentral Internal Medicine Pharmacist to join their Pharmacy team at Baptist Medical Center in Jacksonville, FL. This is a full-time, day shift opportunity, working 7:00 am - 3:30 pm, Monday through Friday and every fourth weekend. The role provides clinical pharmacy coverage for Medical-Surgical and Progressive Care units, collaborating with healthcare teams to optimize medication therapy. Responsibilities include reviewing medication orders, monitoring patient responses, performing medication reconciliation, and providing evidence-based recommendations. The pharmacist manages complex medication regimens, assists with transitions of care, and supports antimicrobial stewardship and medication safety initiatives. They also serve as a medication expert and educational resource for healthcare providers and contribute to quality improvement efforts. Requirements

  • Pharmacist license in the State of Florida.
  • Bachelor of Science Required.
  • Registered Pharmacist - State Board of Pharmacy Licensed.
  • Pharmacist - DOH Licensed.

  • Completed PGY1.

Responsibilities

  • Provides clinical pharmacy coverage for Medical-Surgical and Progressive Care units.

  • Reviews and evaluates medication treatment plans for safety, efficacy, and cost-effectiveness.
  • Monitors patient progress and collaborates on discharge planning and medication education.
  • Enters medication orders accurately and reviews technician work for precision.
  • Serves as a resource for healthcare professionals, offering expert drug therapy consultation.
  • Supervises pharmacy technicians and contributes to departmental committees.
  • Monitors patient therapy including pharmacokinetics services and intervenes when necessary.
