About The Position
Baptist Health is seeking a Decentral Internal Medicine Pharmacist to join their Pharmacy team at Baptist Medical Center in Jacksonville, FL. This is a full-time, day shift opportunity, working 7:00 am - 3:30 pm, Monday through Friday and every fourth weekend. The role provides clinical pharmacy coverage for Medical-Surgical and Progressive Care units, collaborating with healthcare teams to optimize medication therapy. Responsibilities include reviewing medication orders, monitoring patient responses, performing medication reconciliation, and providing evidence-based recommendations. The pharmacist manages complex medication regimens, assists with transitions of care, and supports antimicrobial stewardship and medication safety initiatives. They also serve as a medication expert and educational resource for healthcare providers and contribute to quality improvement efforts. Requirements
- Pharmacist license in the State of Florida.
- Bachelor of Science Required.
- Registered Pharmacist - State Board of Pharmacy Licensed.
- Pharmacist - DOH Licensed.

- Completed PGY1.
Responsibilities
- Provides clinical pharmacy coverage for Medical-Surgical and Progressive Care units.
- Collaborates closely with physicians, nurses, and interdisciplinary care teams to optimize medication therapy.
- Reviews medication orders, monitors patient responses, and performs medication reconciliation.
- Provides evidence-based recommendations to improve patient outcomes.
- Manages complex medication regimens for patients with acute and chronic conditions.
- Assists with transitions of care.
- Supports antimicrobial stewardship and medication safety initiatives.
- Serves as a medication expert and educational resource for healthcare providers.
- Contributes to quality improvement efforts across the inpatient setting.
- Participates in daily interdisciplinary rounds with hospitalists, nurses, and healthcare professionals.
- Reviews and evaluates medication treatment plans for safety, efficacy, and cost-effectiveness.
- Monitors patient progress and collaborates on discharge planning and medication education.
- Enters medication orders accurately and reviews technician work for precision.
- Serves as a resource for healthcare professionals, offering expert drug therapy consultation.
- Supervises pharmacy technicians and contributes to departmental committees.
- Monitors patient therapy including pharmacokinetics services and intervenes when necessary.
