About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist to provide leadership in a specialized healthcare team and patient care to ensure optimal cost-effective drug therapy. This role involves developing pharmacotherapeutic goals, recommending regimens, and creating monitoring plans for individual patient needs. The clinical pharmacist will lead efforts in preventing and reporting medication errors and adverse drug reactions, continuously improving processes and practices to enhance medication therapy safety. This position offers a 7 on/7 off schedule, working overnight hours (7:00 PM - 7:00 AM), primarily supporting patients in various ICUs and the emergency department. Responsibilities include consult coverage, addressing clinical questions, and responding to codes for admitted adult patients. Opportunities for engagement with other shifts, projects, precepting, and committees are available based on individual interests. VUMC is looking for motivated individuals to help expand clinical scope and practice.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year experience
  • Bachelor's Degree (Required)

Nice To Haves

  • PGY2 in ICU or emergency medicine or equivalent experience.
  • PGY1 plus experience in critical care setting.

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
  • Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy.
  • Develops specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient.
  • Leads efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy.
  • Works to continuously improve process and practice in the institution to enhance safety of medication therapy.
  • Primarily supports caring for patients in various ICUs during the overnight hours with some coverage in the emergency department.
  • Has responsibility for consult coverage, clinical questions, and code response for admitted patients in the adult hospital.
