About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist specializing in Malignant Hematology. This role is part of the Outpatient Oncology Pharmacy team and focuses on providing leadership in specialized healthcare, ensuring optimal and cost-effective drug therapy for patients. The pharmacist will develop pharmacotherapeutic goals, recommend treatment regimens, and create monitoring plans tailored to individual patient needs. A key aspect of this position involves leading efforts to prevent and report medication errors and adverse drug reactions, contributing to continuous improvement in medication safety practices within the institution. The Hematology Clinical Pharmacist will provide critical oversight for medication management in the malignant hematology patient population, including chemotherapy/biotherapy orders, new treatment plan builds, supportive care medications, chemotherapy selection and dosing, and adverse event monitoring. This role significantly contributes to financial stewardship, quality and safety initiatives, and research and education goals. The position involves rotation through inpatient and clinic responsibilities, with hours from Monday to Friday, 8:00 am to 4:00 pm, and rotating weekend and holiday coverage (approximately 4 weekends per year). Relocation assistance is provided.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year of experience
  • Bachelor's degree (Required)

Responsibilities

  • Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy.
  • Develops specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient.
  • Leads efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy.
  • Works to continuously improve process and practice in the institution to enhance safety of medication therapy.
  • Provides critical oversight of safe and effective medication management in the malignant hematology patient population.
  • Oversees chemotherapy/biotherapy medication orders and new treatment plan builds, orders for supportive care medications, assists with chemotherapy treatment selection and dosing as well as monitoring and management of adverse events.
  • Contributes substantially to financial stewardship practices, quality and safety initiatives, and research and education goals.
  • Rotates through inpatient and clinic responsibilities.
  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
