Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist specializing in Malignant Hematology. This role involves providing leadership within a specialized healthcare team to ensure optimal and cost-effective drug therapy for patients. Key responsibilities include developing pharmacotherapeutic goals, recommending treatment regimens, and creating monitoring plans tailored to individual patient needs. The Clinical Pharmacist will also lead efforts in preventing and reporting medication errors and adverse drug reactions, contributing to continuous improvement in medication safety practices within the institution. This position offers critical oversight of medication management for malignant hematology patients, including chemotherapy/biotherapy orders, new treatment plan builds, supportive care medications, chemotherapy selection and dosing, and adverse event management. The role also contributes to financial stewardship, quality and safety initiatives, and research and education goals. The pharmacist will rotate through inpatient and clinic responsibilities, with hours from Monday to Friday, 8:00 am to 4:00 pm, and rotating weekend and holiday coverage approximately four weekends per year.
