Clinical Outreach Liaison

Amae Health•Boston, MA

About The Position

Amae Health is seeking a relationship-oriented Community Liaison to drive referral growth in the Boston region. This hybrid role combines territory-building business development with hands-on patient advocacy. The liaison will develop and manage referral relationships with hospital systems, emergency departments, discharge planners, community mental health centers, and outpatient providers, while personally supporting patients through warm handoffs into care. The role requires a home address within the Boston region and regular travel throughout the territory, including daily field work. The liaison will build and maintain a pipeline of qualified referrals, activate new referral sources, deepen relationships across health systems, and re-engage inactive partners. They will also help patients and families navigate transitions into care by meeting them in hospitals, at home, or in the community as needed. The position owns CRM documentation, quarterly market planning, and weekly reporting on pipeline health, conversion, and account risks. Performance is measured by qualified referral volume, referral-to-intake conversion, new active referral sources, and CRM completeness.
