Amae Health•Boston, MA
•$80,000 - $100,000•Hybrid

About The Position

Amae Health is seeking a driven, relationship-oriented Community Liaison to own referral growth in the Boston region. This full-time, hybrid role combines territory-building business development with hands-on patient advocacy. The liaison develops and manages relationships with hospital systems, emergency departments, discharge planners, and outpatient providers to create a consistent pipeline of qualified referrals. They also personally support referred patients through warm handoffs, meeting them in hospitals, at home, or in the community to help ensure the transition into care succeeds. The position requires a home address within the Boston region and regular travel throughout the territory. The role is organized around referral partner development and territory growth (60%), warm handoff and patient-facing support (20%), and pipeline management and reporting (20%). Success is measured by qualified referrals per month, referral-to-intake conversion rate, net-new active referral sources per quarter, and CRM/documentation completeness. Amae Health is a venture-backed Public Benefit Corporation providing specialty outpatient care for people with severe mental illness through high-acuity, in-person clinics. Its integrated model brings psychiatry, therapy, primary care, dietitians, and community support together. The company also develops clinical protocols, measurement approaches, and an AI-driven data platform for severe mental illness.
