Requirements

  • Requires advanced clinical skills; with appropriate credentialing based on the patient population (this is often reflected in an advance degree in the field of specialization; i . e . MSN or RRT.
  • Requires knowledge of research/methodologies.
  • Requires strong interpersonal; problem solving and analytical skills.
  • Requires strong organizational skills.
  • Requires effective communication and conflict resolution skills.
  • Requires ability to lead and delegate effectively.
  • Requires strong clinical expertise within assigned patient population.
  • Requires the ability to educate patient populations and influence behaviors.
  • Requires the ability to educate staff and serve in a mentoring role.
  • Requires strong computer skills and proficiency in applicable software (spreadsheet; database; etc . ).

Responsibilities

  • Design and Innovate: Create, implement, and evaluate programs tailored for outcomes management across designated patient populations.
  • Educate and Empower: Provide education, advanced consultation, and management support for key service lines, ensuring your peers and patients are informed and empowered.
  • Collaborate for Excellence: Team up with the patient care team to guarantee top-notch care while effectively managing clinical costs and enhancing service outcomes.
  • Drive Quality Improvement: Utilize evidence-based practices to assess care pathways, uncovering opportunities for quality enhancement and cost savings.
  • Lead and Evaluate: Spearhead initiatives focused on resource utilization and develop data-driven evaluations for continuous performance improvement.
