Requirements

Requires advanced clinical skills; with appropriate credentialing based on the patient population (this is often reflected in an advance degree in the field of specialization; i.e. MSN or RRT.

Responsibilities

Design, implement, and evaluate programs tailored for outcomes management across designated patient populations.
Provide education, advanced consultation, and management support for key service lines.
Collaborate with the patient care team to ensure top-notch care while managing clinical costs and enhancing service outcomes.
Utilize evidence-based practices to assess care pathways, identifying opportunities for quality enhancement and cost savings.
Spearhead initiatives focused on resource utilization and develop data-driven evaluations for continuous performance improvement.
