This position provides in-person guidance and support to providers, care teams, and affiliates in clinical integration initiatives. The role aims to support the quadruple aim and enable the organization to thrive in a value-based healthcare model. Key responsibilities include maintaining a deep understanding of practice drivers, translating data into actionable insights, collaboratively identifying improvement steps using a Plan-Do-Study-Act (PDSA) model, and coaching care teams in process improvements, clinical integration, and outcomes.
