About The Position

The Clinical Liaison is responsible for conducting assessments for all per MDS schedule, support documentation and social care plans geared to restoring residents/patients to their optimum level. Will work with incoming residents in collaboration with other departments to facilitate a smooth transition into the community. This position requires excellent clinical strengths, communication skills, and detail orientation. Supports residents and their families to utilize the resources internally and in the community, particularly concerning Medicare and Medicaid. This position promotes a positive, calm, and effective work environment that contributes to teamwork. The Clinical Liaison position requires excellent communication skills with team members, residents and family members as well as a high level of patience and compassion for others.

Requirements

  • Must possess current RN or LPN license in Ohio.
  • Experience in a medical facility, senior living facility preferred, where the team approach to treatment is utilized.
  • Microsoft Office proficiency. AOD, HENS system, and care tracker knowledge preferred.
  • Strength in developing and maintaining positive, professional relationships with residents, families, and interdisciplinary team.
  • Fluency in reading, writing, and speaking the English language
  • Ability to pass a pre-employment background check
  • Ability to pass a pre-employment drug screen

Responsibilities

  • Attends and participates in clinical meetings for continuity of care.
  • Attends and participates in leadership meetings.
  • Achieve or exceed budgeted occupancy in skilled nursing.
  • Supports CMS Quality Measure outcomes.
  • Complete resident interviews and assessments.
  • Provides consultation to members of staff and coordination with community agencies etc. in an effort to solve issues and meet needs of residents and ensure appropriate care.
  • Discharge planning.
  • Coordinates insurance appeals.
  • Assists residents and their families with personal and environmental difficulties.
  • Consults with physician and other members of the care team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient's/resident's health issues.
  • Organizes and plans discharge process for long term care residents and for short term/skilled residents moving to different levels of care.
  • Develop Baseline, Social Services, and Discharge care plan.
  • Assist with clerical tasks.
  • Schedules and participates in care conferences, as part of interdisciplinary team.
  • Oversee and complete documentation required for Part A and Part B billing; i.e., Notice of Medicare Non-Coverage, Advanced Beneficiary Notice.
  • Oversees Health Services staff in the absence of the Nurse Manager. Acts as whole house manager of duty at community. Directs day-to-day job function duties of resident assistants (STNAs, RAs) and Nurses (LPNs and RNs). Makes staffing decisions.
  • Oversees and monitors care of the residents.
  • Conducts resident and associate injury investigations and determines appropriate resolution, handles injury documentation at time of incident.
  • Makes written and oral reports/recommendations to the Director.
  • Participates in State survey.
  • Acts as charge nurse as needed which includes giving and receiving the nursing report upon the start and end of each shift, providing individualized attention, implements plan of care, dispenses medication, takes vital signs, and maintains and manages mid-line pic line IVs as ordered. Takes cultures and specimens. Reviews and documents activities via MARS and TARS and AOD.
  • Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Admissions, transfers, and discharge functions as assigned.
  • Communicates with Doctor(s), families, and other health professionals.
  • Verify payer sources information for all admissions (Medicare, HMO, insurance, private pay, etc.)
  • Collaborate with Nurse Manger on all admissions to establish goals for patient care, participate in care conferences, utilization review weekly meetings and discharge planning.
  • Collaborates with Clinic Nurse for prospective residents; conduct in-office and in-home visits, assessments and presentations as needed, review and obtain all required admissions paperwork.
  • Acquire insurance Pre-Certifications for incoming patients and provide updates for recertification.
  • Issue daily updates on ACO and other partnerships through third party administrator.
  • Participate in weekend rotation, serving as clinical supervisor for all areas of campus.
  • Provide departmental back-up, as needed, in campus SNF, Memory Support, Assisted Living, or MDS areas.
  • Serve as main contact with hospitals and other healthcare professions to facilitate referrals, including managing situations that arise outside normal business hours
