About The Position

Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996, caring for patients with serious and terminal illnesses throughout North Central Texas. Their mission is to provide compassionate care for those living with an illness while supporting those caring for a loved one. The Clinical Nurse Liaison - RN serves as the hospice expert, coordinating services and facilitating the transition of patients from the hospital to hospice services. This role involves developing long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.

Requirements

  • Registered RN in the State of Texas, or LMSW required
  • Nursing experience or medical training required (ability to read charts, know medications, etc.)
  • Marketing experience required
  • Proficient skills using a computer are required

Nice To Haves

  • 1-3 years of experience in healthcare marketing preferred
  • Hospice experience strongly preferred

Responsibilities

  • Respond to referral sources in a timely manner; evaluate patient appropriateness for hospice; meet with families/patients to discuss goals and explain services of hospice.
  • Complete required paperwork with patients/families; facilitate the patient discharge by working with hospital staff, DME, medications, hospice physicians, and hospital staff.
  • Educate hospital staff and follow up on pending patients; round daily to be available to staff and patients for questions and to build relationships.
  • Communicate and provide daily feedback to the admission staff on referrals.
  • Completes thorough documentation of hospice eligibility in the required time frame.
  • Through customer service and leveraging relationships, become the “hospice of choice” whereby increasing hospital referrals.
  • Attend and participate in routine staff meetings in person or via media provided as directed.
  • Participate in and/or provide in-service training, attend various hospital activities, and perform other duties as required or assigned.
