Requirements

  • Currently certified, credentialed, or licensed healthcare professional.
  • Proven experience managing a healthcare team.
  • Strong knowledge of healthcare terminology, chronic conditions, and care coordination.
  • Experience with Electronic Medical Records (EMR) software.
  • Proficiency with Microsoft Office Suite (Outlook, Excel, Word) and Zoom.
  • Ability to develop trusting relationships with staff, patients, and partner practices.
  • Ability to thrive in fast-paced, dynamic environments.
  • Strong communication, organizing, multitasking, and time management skills.
  • Ability to pass drug screening and background check.

Nice To Haves

  • Previous management or supervisory experience in healthcare settings.
  • Experience with performance management and team development.
  • Familiarity with care coordination platforms or EMR systems.
  • Background in chronic care management or similar care models.
  • Experience with quality improvement initiatives.

Responsibilities

  • Lead and oversee the performance of Care Coordinators within your region.
  • Create and maintain a workplace culture consistent with the organization’s mission, vision, and values.
  • Empower employees to take responsibility for their roles and goals.
  • Foster teamwork and unity within assigned teams.
  • Monitor daily metrics and investigate causes of lowered productivity.
  • Provide coaching, guidance, and development to care coordinators.
  • Conduct performance reviews (90-day and annual).
  • Schedule weekly 1:1s with Care Coordinators.
  • Administer corrective action forms and performance improvement plans in collaboration with HR and Clinical Directors.
  • Lead team meetings to review performance, identify opportunities for improvement, and enhance transparency.
  • Lead opportunity for improvement (OFI) meetings.
  • Participate in monthly planning meetings with Clinical Directors to review production, patient allocation, and quality metrics.
  • Participate in all scheduled director and clinical manager calls and meetings.
  • Manage staffing and scheduling for your team.
  • Coordinate EMR access for new hires.
  • Maintain timecards and approve PTO requests in a timely manner.
  • Partner with HR to resolve conflicts and provide care coordinators with support.
  • Escalate issues to Clinical Directors as needed.
