About The Position

Interim HealthCare of Oklahoma City is hiring a Clinical Liaison to meet patients before discharge, educate families on home health and hospice, and ensure a smooth transition from hospital to home. This role offers autonomy in managing relationships and being the initial point of contact for patients from the team. The position involves travel between hospitals, rehab facilities, and skilled nursing facilities across the OKC metro area.

Requirements

• Active Oklahoma LPN license
• At least 2 years of LPN experience
• Previous home health experience required
• Valid driver's license, reliable transportation, and current auto insurance
• Current CPR certification

Nice To Haves

• Liaison, hospital, or hospice experience
• Established relationships with OKC-area hospitals, case managers, or discharge planners
• Working knowledge of Medicare home health and hospice eligibility criteria
• Confident, warm communicator comfortable at the bedside and with referral sources

Responsibilities

• Visit patients at the bedside in hospitals, rehab centers, and skilled nursing facilities to explain home health and hospice services.
• Review clinical records and gather information to confirm patient appropriateness for home health or hospice.
• Partner with discharge planners, case managers, social workers, and physicians to ensure complete and accurate referrals.
• Coordinate with the intake team to ensure timely start of care at home.
• Answer patient and family questions and build confidence regarding post-discharge care.
• Build and maintain relationships with referral sources across the OKC metro.
• Maintain accurate, HIPAA-compliant records of referral activity.
