About The Position

Clinical Liaison (RN / RRT) at Select Specialty Hospital Critical Illness Recovery Hospital (LTACH). This role involves advocating for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you will coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care. This is a full-time position, Monday-Friday, 8 am-5 pm, with a competitive salary plus uncapped monthly bonus incentives and an extensive onboarding & training program. Requirements

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Ability to complete clinical assessments of medically complex patients.

Nice To Haves

  • Will consider other designations like LPN, PT, OT etc., upon further review.
  • Demonstrated success in patient advocacy.
  • Superior communication and relationship building skills.
  • Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.

Responsibilities

  • Work with physicians, discharge planners and patient's families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge and problem-solving skills.
  • Perform clinical assessments of referred patients, then counsel with patient's families to assist them in making the best care decisions for their loved one.
  • Partner with hospital case managers and physicians to create a smooth pre-admissions and transfer processes through removing payer-related and operational barriers for the patients getting into our highly effective level of care.
