About The Position

The Clinical Liaison is responsible for the delivery and facilitation of health and health related services including medical care, health information services and self-care education via telecommunications. The clinical liaison will serve as a key point of contact for patients calling into internal medicine practice, handling patient phone inquiries, guiding patients to the appropriate care, assisting with prescription refills, and coordinating closely with providers to ensure seamless care transitions.

Requirements

  • Current, active NYS RN license
  • Certification as a Medical Assistant (CMA) preferred
  • Minimum of 1-2 years of experience in a clinical or patient coordination role
  • Experience in a call center, triage, or direct patient care is a plus
  • Familiarity with internal medicine practices and workflows preferred
  • Certified Medical Assistant (CMA) or Registered Nurse (RN) preferred
  • Previous experience in a patient-facing role, especially in internal medicine or primary care
  • Excellent telephone etiquette and communication skills
  • Ability to triage patient needs and escalate care as needed
  • Familiarity with EHR systems and basic office software
  • Compassionate, organized, and able to multitask in a fast-paced environment
  • Strong organizational and multitasking skills
  • Critical thinking and problem-solving abilities

Nice To Haves

  • Associate or bachelor’s degree in nursing, healthcare administration, or a related field preferred.

Responsibilities

  • Answer incoming patient calls promptly and professionally.
  • Assess patient needs, guide them to the right care, and address questions about symptoms, appointments, and services.
  • Process prescription refill requests, ensuring appropriate approvals and documentation.
  • Refills prescriptions if appropriate, under the direction of the provider.
  • Coordinate with providers to relay patient concerns and follow up on care plans.
  • Maintain accurate documentation in the electronic health record (EHR).
  • Track patient concerns and coordinate follow-ups, ensuring continuity of care.
  • Coordinate with the providers to discuss patient’s test results. This includes lab work, radiology and/or specialist reports.
  • Obtain information from patients requesting forms to be filled out.
  • Prepare and fill out forms for completion and signature.
  • Process referrals or prior authorizations as needed.
  • Assist with answering patients’ questions they may have about a previous or future appointment, or other medical issues.
  • Assist with recalls to patients reminding them of the need to schedule well visits, or for follow-up appointments.
  • Assist the nurses/MAs on the floor as needed.
  • The Nurse Clinical Liaison must be compliant with and cooperate fully with Ezra Medical Center’s compliance program, including its standards of conduct and policies and procedures. Such compliance will be an element considered as part of the Nurses Clinical Liaison’s regular performance evaluation. Failure to comply with EMC’s written standards, which includes the failure to report any conduct or event that potentially violates legal or compliance requirements, will be met by the enforcement of disciplinary action, up to and including possible termination in accordance with Ezra’s compliance program.
